What is the vial of life program?

The vial of life is a form that is filled out by you and stored in the top shelf of your refridgerator.  This form includes information such as your name, birthdate, medical history, medications, allergies and other helpful information.  You can also include a copy of a recent EKG reading or a current picture of yourself.

Why should I get a vial of life?

The vial of life is very helpful for emergency responders who arrive at your home during a medical emergency.  If you are unable to speak, or provide this important information, emergency responders can locate this information immediately.  This provides crucial information without the delay of tracking down each piece of information.

Why is it stored in the refridgerator?

This is the universal location to store the vial of life.  All emergency responders know to look there for it.  You can also obtain a second vial of life and keep it stored in your car.

What do I do now?

  1. Visit your local ambulance base and request your vial of life.
  2. Fill out the requested information.
  3. Place the form in the bottle that is provided.
  4. Place the bottle on the top shelf of your refridgerator.
  5. Place the vial of life decal that is provided in your front window or door.
